The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following South Sound health care providers.
In March, the Dental Hygiene Program charged Karen A. Lockhart with unprofessional conduct. She allegedly billed for services not provided, resulting in an overpayment of at least $14,500.
In March, the Nursing Commission charged licensed practical nurse Catherine Margaret Ervin with unprofessional conduct. She allegedly failed to keep adequate records of withdrawal and administration of medications to patients.
In March, the Nursing Commission charged registered nurse Jolene Marie Jackson with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her license.
In March, the Emergency Medical Services and Trauma Systems Program ended the probation order against paramedic Michael James Wilson.
