The Department of Social and Health Service has notified Tacoma’s A Fresh Beginning that it is revoking the treatment facility’s certification.

The revocation action comes after the Department of Health found that Clarence F. Farmer, administrator and owner of A Fresh Beginning — formerly called A Change Counseling — had committed unprofessional conduct and suspended his credential as a chemical dependency professional for at least three years.

DSHS, which licenses and certifies treatment programs for chemical dependency, community mental health and problem gambling, is required to revoke a provider’s certification when a current owner or administrator has committed unprofessional conduct. Farmer has 28 days from receipt of the notice, sent to him via certified mail on Oct. 9, to contest this decision.
